1. The Diabeter clinics are organized as an Integrated Practice Unit with a multidisciplinary team, focusing solely on diabetes care and working toward the common goal of measuring and optimizing our patients outcomes.

2. At Diabeter, baseline data, outcomes and costs are measured and analyzed for every patient in order to maximize the value of care for the patient as efficiently as possible.

3. Diabeter is now moving towards complete annual bundled payments and a payment per outcome model in order to further incentivize improved patient value and outcomes instead of volume.

4. Diabeter has concentrated and integrated care into a focused network of five locations that function as an IPU. This concentration allows developing increased knowledge, excellence and better use of resources thereby adding value for patients, providers and the healthcare system as a whole.

5. Since 2006, further 4 clinics have been opened in the Netherlands and the Diabeter model is expanding internationally through partnership or acquisition, in order to deliver better care and outcomes to patients globally.

6. An integral part in Diabeter's care model is the eHealth system, VCare, collecting, tracking and analyzing real-time patient data to provide optimized and individualized care. Data overview and analytics are communicated to the patients through ther@pymail.
